Bookings must be made at least 2 working days in advance.
Once your booking request is successful, a confirmation email and relevant details will be sent to your email address.
The assessment experience and report retrieval must be completed via the LIVE4WELL App. Upon arrival at the LIVE4WELL Intelligent Health Assessment Centre, please download the LIVE4WELL App and register as a member (on-site staff will be available to assist you).
